The randomization precedes the surgery. So, the arm A, the axillary
dissection arm, actually gets a sentinel node also. So everybody
gets a sentinel node, but the arm A or group 1, that gets randomized
to axillary dissection gets a sentinel node and an axillary dissection.
And the reason that we are doing that is because in our validation
study — that was the one published in New England Journal
— demonstrated that 3% of the patients who had pathologically
positive nodes had them exclusively outside the axilla. Knowing
that, to take a treatment arm and not give them what we know might
offer them an improved benefit would be questionable. It would be
hard to justify withholding, now, sentinel node when we know it
enhances our ability to find things, even if we are going to do
an axillary dissection.
