Dr. Krag: The simplest way to view that is that the NASBP study
is a node negative trial and the ACOG group is a node positive trial.
So, patients that have a pathologically positive sentinel node will
be randomized to axillary dissection or no axillary dissection.
There
is a pathology study, thats an ancillary pathology study thats
really very similar to both studies and are doing nearly the same
in terms of looking at the nodes. Its done blinded and so
forth, immunohistochemistries. Theyre also doing a bone marrow
study, as well, in a portion of these patients, so theyll
get their bone marrow aspirated and checked as a prognostic factor.

Love: What are your thoughts about that study?
Dr.
Krag: I think that it will be good to have two different studies.
There should be identical data on the immunohistochemistry analysis
and nodes, and Ill feel a lot better if they are. The bone
marrow data also I think will be valuable, particularly theres
more and more articles coming out, one just out in The New England
Journal on bone marrow analysis from Europe. And we really havent
done much of these kinds of studies in the United States, not that
the biology is any different but it will be nice to confirm in a
larger group of patients prospectively the meaning of this. It would
be nice though to have something other than any of these prognostic
factors because they are all applied to a population of patients
and do not apply to any given individual. Nobody can look at one
individual and say, "This is it for you," and thats really
what we should target our goals at, is developing something that
would do for that individual patient and we may have that in the
near future.
