I
think the idea of Mel Silverstein’s that you have to treat
this as a Halstedian disease now is the wrong way to go. DCIS is
a disorder of growth and we have to control it with growth controlling
drugs. And here I try to get people to think of the margin between
atypical hyperplasias and DCIS because it is a continuum. It is
even more difficult to find the difference between those two entities
than it is at the other end of the spectrum where you define the
difference between DCIS and invasive cancer. Even there there’s
some arguments, but at the lower end of that scale we’re dealing
with an entity like atypical hyperplasias where we all agree it’s
not a surgical disease and we’re all thinking of tamoxifen
and newer agents. At the upper end of that scale or in the middle
of that entity DCIS, we tend to think of it as cancer and I don’t
think that’s sensible. I think we can shift our thinking toward
the left and address this disease as non-surgical. As I say that
I don’t want to get into an argument where I don’t think
surgery has a place. I do think surgery has a place now, but I think
it’s only because we haven’t got the real treatment that
we need.
