If
we agree that ductal carcinoma in situ is a proliferation of abnormal
cells that is contained within the basement membrane, we know that
in the pre-mammographic era, DCIS was uncommon and was overwhelmingly
cured by mastectomy alone.
Now,
I think its important to recognize that as many people start
to focus on so-called bad DCIS, high histologic grade, comedo lesions,
large DCIS and while its true that all of those factors
are associated with a higher incidence of invasive carcinoma within
the lesion once you have invasive carcinoma, you dont
have DCIS anymore.
And,
if youre talking about pure DCIS, there is virtually no such
thing as bad DCIS when you have a disease with a cause specific
survival that is routinely over 96 or 97 percent.
