If
we agree that ductal carcinoma in situ is a proliferation of abnormal
cells that is contained within the basement membrane, we know that
in the pre-mammographic era, DCIS was uncommon and was overwhelmingly
cured by mastectomy alone.
Now,
I think it’s important to recognize that as many people start
to focus on so-called bad DCIS, high histologic grade, comedo lesions,
large DCIS – and while it’s true that all of those factors
are associated with a higher incidence of invasive carcinoma within
the lesion – once you have invasive carcinoma, you don’t
have DCIS anymore.
And,
if you’re talking about pure DCIS, there is virtually no such
thing as bad DCIS when you have a disease with a cause specific
survival that is routinely over 96 or 97 percent.
