The
way I summarize my thinking about DCIS at this point in time is
that our ability to understand the risk of ipsilateral breast tumor
recurrence is steadily improving as we get good data from prospective,
controlled trials where patients have been treated in the same fashion.
That at the present time, when making decisions about the intensity
of treatment, we should attempt to tailor those both to what we
believe the patients individual level of risk is. And things
that appear to predict that risk are age, perhaps the histologic
grade of the lesion, the ability to at least excise it to a negative
margin, and, most importantly, the patients attitude toward
minor benefits from treatments with potential toxicities. And finally,
we need to look towards our colleagues in the laboratory to give
us better markers for the ultimate risk of breast cancer death other
than waiting for invasive breast occurrences to occur.
