Dr.
Giuliano: I do not routinely do mastectomy for DCIS but we treat
patients with DCIS with breast conserving surgery, usually with
radiation therapy and now frequently with adjuvant tamoxifen.
Dr.
Love: What are your thoughts about tamoxifen?
Dr.
Giuliano: I think there’s two reasons to give adjuvant tamoxifen
in DCIS. One is it decreases the local recurrence within the breast
and two, you’re less likely to get a contralateral breast cancer.
I think those are compelling reasons. Of course, you have to individualize
and speak with the patient about the risks of tamoxifen, its not
risk-free. It’s a very safe drug and I think it can be used
in many women with DCIS.

Love: Do you yourself use tamoxifen or do you usually refer the
patient to an oncologist?
Dr.
Giuliano: It depends on the patient. I frequently prescribe the
tamoxifen myself. Many patients want to see a medical oncologist,
and it’s valuable to hear other opinions. This is not a straight-forward
area, there’s controversy and frequently they’ll get other
opinions and see medical oncologists.
