Papers have been written saying that, at this
point in time, sentinel node mapping is not the standard care. To
me that means that, across the nation everybody doing it, we are
not there yet. But, certainly, in the hands of people who have a
lot of experience with it, who validated their own experience, certainly
it is an acceptable standard of care. The reason I say that is that
the anatomic hypothesis of the existence of a sentinel node has
been answered, it is validated, there is no question in anybody’s
mind. So the challenge is, "Am I finding the sentinel lymph node,
am I actually identifying it?" What we are really talking about,
at the core of this, is the elimination or the reduction in removing
nodes unnecessarily. There can be no benefit from removing negative
lymph nodes. So, the decision point is, "If I know how to do this,
and I believe in it, and I have validated my experience, should
I offer it to my patients as a standard of care?" The physicians
that are involved with the clinical trials end of this feel that
it is incumbent at this point to put patients on trial, to do this
on a trial setting. The trials are important, I certainly support
them, but I think that if the surgeon out there learns to do this
and validates it, that it certainly can be offered.
